 Applying to the LPN to RN Program

The Nursing Program at Heritage University has admission requirements above those necessary for general admission to the university. Application must be made to both the program and the university. In addition to the prerequisites listed below students must meet all of the legal requirements of the institutions or agencies where clinicals will occur. This includes a Verified Credentials National Background check upon admission and a Washington State Patrol criminal background check each semester. Health insurance and individual student malpractice insurance are required, as are health record documentation of required immunizations and medical test certifications.

 
Admission Requirements
  • Must be a Licensed Practical Nurse
  • LPN to RN Transition Test (testing dates: May 12, 9:00 a.m. - 4:00 p.m. or May 17, 9:00 a.m. - 4:00 p.m.)
  • Comprehensive Predictor Practical Nursing NCLEX Assessment (only required if you have been out of school for five years or more)
  • Verified Credentials national background check and Washington State Patrol clearance with the absence of behaviors or attitudes that would place patients at risk (completed each semester)
  • Current immunizations and certifications
  • Completed the following pre-requisite requirements with a  "C" or better
    ENG 101  (English Comp I)
    MATH 221 (Intro to Probability and Statistics)
    CHEM 101 (Intro to Chemistry, with lab)  
    BIOL 110/ BIOL 111 (General Biology I and II )
    BIOL 211/BIOL 212 (Anatomy and Physiology)
    BIOL 220 (Principals of Microbiology)
    PSY 101 (Intro to Psychology)
    SOC 101 (Intro to Sociology)
    HPER 210 (Nutrition)

     

Applying to Heritage University 

Applying to the LPN to RN Program
Submit all LPN to RN program application materials collectively to: Heritage University, Arts and Sciences Center, Nursing Program, 3240 Fort Road, Toppenish, WA 98948.

  • Complete and submit the LPN to RN Program Application
  • Submit a 1-3 page personal mission statement describing why you want to be a nurse, include life experiences, personal characteristics, work experience, etc.
  • Two (2) personal recommendations using the nursing program recommendation form 
  • Unofficial transcripts from any other college, university or junior college you have attended (this is in addition to the transcripts submitted for admission into the university)

Candidate Evaluation
Successful applicants will be selected by an application review committee consisting of Heritage faculty and staff.  Applicants will be evaluated based on the following:

  • Completion of the pre-requisites courses
  • The content of the personal mission statement
  • The content of the two personal recommendations
  • Grade point average and the absence of any continuing academic problems
  • Acceptable Washington State Patrol background check
  • LPN to RN Transition Test
  • Comprehensive Predictor Practical Nursing NCLEX Assessment test (if required)
  • Candidate interview
