It is the responsibility of the student to provide the Nursing office with proper documentation of the required immunizations and to see that the documentation remains current.
Tetanus and diphtheria Combined (Td)
Tetanus injection in the last 10 years.
Measles, Mumps immunization
At least once in a lifetime.
Rubella and Rubeola Titer
Must have proof of a positive titer (blood test) done within the last 5 – 10 years.
Varicella Vaccine (chicken pox)
Proof of inoculation, titer or documented history that student has contracted chicken-pox.
All students must have a complete primary series of polio vaccine and have documentation from a health care provider or a letter from a family member that series was received.
Hepatitis B (HBV): a series of 3 injections
1st Initial injection
2nd one month after the 1st
3rd six months after the 1st
Must have documentation of completion of the first two injections prior to the first clinical lab. The 3rd injection must be completed in a timely manner.
Hepatitis A (HAV) series of 2 doses
1st dose – initial dose
2nd dose – 6 months after 1st
Medical Test and Certification Requirements
Tuberculin Skin Test (PPD)
Required once a year. If the student does not have a documented negative skin test result within the preceding 12 months, a two-step baseline must be used. Students with a positive PPD must have a chest x-ray and provide documentation that demonstrates no active Tuberculosis.
Documentation of 7 hours of certified AIDS/HIV training.

Current CPR card
